Early integration creates opportunities to understand what matters most to the person\, including their values\, beliefs\, wishes\, and end-of-life choices\, and to reflect these through advance care planning and individualized\, interdisciplinary care. \n\n\n\n\n\n This one-hour webinar will introduce CARF’s Palliative and End-of-Life Care Specialty Designation and explore how the standards support person- and family-centred palliative and end-of-life care across Aging Services settings.
